同学你好，College Report是转学申请中比较常见的一个表格。可以在Common Application系统里面下载，之后找你原学校的advisor或者office of registrar填写，里面主要是你的GPA，是否有违纪行为等信息。填写完毕之后记得根据你申请院校的要求上传或者邮寄哦 ：）
